The cap is BRAND NEW IN PACKAGE.

The base was taken out of package but never used. The base is style CP601 which according to vortex fits yamaha.

Cap is universal fit.

Please double check for fitment for your bike. I do not want to be responsible if you order and it doesn't fit.

Is there any hardware missing, like the bolts to mount the base?

No bolts to base. I'm not sure if it came with them... but you could use the hardware to your original cap. Or use hex bolts (M6?)from hardware store. Cost is less than a buck.
