From the wikipedia:
A point of confusion can come from the difference in the microSD and the newer micro SDHC format. The SD and SDHC share a same form factor, but not all devices are compatible with the newer high capacity format. This is even true with devices that have been developed by SanDisk such as their e200 series of MP3 players. Using 3rd party firmware however, SDHC reading capabilities can be possible, but requires an external writer
pick up an SD card reader and you should be all set

newer HDSD card?
around the time that the 4gb barrier was crossed for SD devices, they came out with a format variant call sdhd or sdhc.. or some crap like that.
if your reader can't read it, you MAY if you're really lucky be able to update the firmware for the reader to recognize the new format.
if a firmware update doesn't work then you need to buy a new reader.

I had the same problem. I ended up buying a newer card reader from NewEgg for like $20. Now my 4gb SD card works mint. Also another thing that threw me for a loop is apparently you can only store up to around 600mb on the card before it throws an error. In order to add more files to the card you have to create a new folder and and store everything in there. You can't just store all 4gb on the top level.
I had the same issue and there are some fixes for it, mine anyway.
My PC, HP nx9420 notebook needed a new driver for the reader to see the card. That was it.
This was for a TI card reader btw.

Here is another fix that works. I have used this on an XP and Vista PC.
I copied and pasted it from the HP support forums.
This registry trick may help:
1. Backup your registry
2. Run regedit. Key you want to edit is located in:
HKEY_LOCAL_MACHINE\SYSTEM\ControlSet001\Services\tifm21\Parameters
Original Key Value: SDParam = 16 (Decimal)
New Key Value: SDParam = 1
3. Reboot for new setting to take effect.
