Paging hqp on the suit....
Sidi's are not really wide so I would go with some smx-R's (I have both) as far as a suit I would try a 42/52 and I have an A-star suit you can try on if you want.
I've used Joe Rocket and A* boots and have had no problems. My friends Sidi's were too tight on me, too.
The A* boots have alot of room if you have wide feet. I love my SMX-R's, but they are a tad wide. The new TCX are also a bit wider than Sidi's, I have a pair of those too!
As far as suits go, an A* 42 is going to be pretty tight on your shoulders, though a Joe Rocket might fit you a bit better. A* is generally a bit longer in the legs and slim up top (Euro fit, same as Dainese, Spidi, etc.). A suit like JR or Komodo(www.komodogear.com) might fit a bit better considering your dimensions. Komodo suits are all like 30% off right now too. Tecknic makes a really nice suit for good money, but they are typically a size smaller than other major brands (a 42 fits like a 40) check out www.sportbiketrackgear.com for Tecknic and A*stuff, good reviews on gear, and excellent people to work with.

I have wider feet and my A*s fit pretty good and there for sale now too.
A* SMX-4 are pretty wide
I have a narrow foot and they're a little floppy for me...

I went with the Alpinestars Supertech-R over the TCX. Both are nice boots and are wider than my Sidis, but i found the TCX was a bit larger/looser in the ankle (I have skinny ankles/calf).
AStar SMX+
I've got a small (9) wide foot and like the fit of them. I've picked up another pair of the same thing for when these die.
