That's just the Q1 form from the DMV. It functions like a bill of sale and can be used when the title itself is filled out. IE, original seller A sells the bike to party B. Party B signs the title, but never registers the bike. B can fill out the Q1 form and have buyer C sign it as well for the full paper trail.

I am inquiring about that fz. Pretty clear he doesn't have a title but no mention of an old reg. I have no desire to put another bike through Vermont if I can help it.

I think mass adds a bit of complexity to the process. They will make you get it notorized at a minimum and then generally a bit more. They may even require a new title from the seller. You never really know what it will take to clear up a simple mistake. It's usually a painful process.
